Student Packets
Open and print the following documents. Complete them and bring them with you the first day of the course. All students must print and complete the Student Instructor Agreement. Then print and complete the IANTD, NSS-CDS, PADI, SDI or TDI Student Packet, depending on which course(s) you are enrolled in. Download, read, and complete the form below appropriate for the course you are signed up for. If you aren’t sure which agency, please contact us. Please read the instructions carefully. You must write “YES” or “NO” for each of the medical conditions asked about in most of the student packets. If you answer “YES” to any of the questions, please also download the Physician Release form and bring it to your physician for a signature.
